• Infinity MileageLands Enrollement Application Form(fileSize, fileExtension)

  Please download the membership application form and simply fill it out and mail/fax to the Infinity MileageLands Service Center.

• Infinity MileageLands Hotel Accommodation Voucher Application Form(fileSize, fileExtension)

  When members fulfill the requirements for ordering a hotel accommodation voucher with valid electronic miles earned under a same account number, members or clients can fill out the Hotel Accommodation Voucher Application Form.

• Infinity MileageLands Member Data Change Application Form(fileSize, fileExtension)

  After completing this application form, please fax or mail to local Infinity MileageLands Service Center.

• Membership Termination Application Form(fileSize, fileExtension)

  After completing this application form, please fax or mail to local Infinity MileageLands Service Center.

• Infinity MileageLands Nominee Register Application Form(fileSize, fileExtension)

  To apply for nominee registration, members can fill out the Nominee Register Application Form and send/fax it to Infinity MileageLands service center.

• Infinity MileageLands Online Service Form(fileSize, fileExtension)

  For easy to apply for nominee registration or Mileage Transfer via EVA Air's website, members need to fill out the On-line Service Form.

• Infinity MileageLands Transfer/Return Application Form(fileSize, fileExtension)

  To apply for Mileage / Upgrade Certificate Transfer members can fill out the Transfer/Return Application Form (Please refer to Appendix 2), and send/fax it to Infinity MileageLands service center.

• Infinity MileageLands Journal / Commodities Application Form(fileSize, fileExtension)

  All these journal are in Chinese version and some of commodities could only be mailed domestically in Taiwan (Electronic magazines/Newspaper excluded )

• Infinity MileageLands Flight Simulator Experience Application Form(fileSize, fileExtension)

  Exclusively for Diamond and Gold Card members. Please fill out the EVA Air Flight Simulator Experience Application Form and fax it to 886-3-351-0005. Note that the required 100,000 miles will be deducted from your account after your flight simulator session has been confirmed.

• Indemnity of Payment(fileSize, fileExtension)

  To ensure the financial security of credit card holders, EVA Air requires that credit cards used to make ticket purchases are available for verification before passengers are accepted for travel.

• Cabin Baggage Declaration Form(fileSize, fileExtension)

  Apply for booking Cabin Baggage use. After completing this form, please fax to EVA Air Reservation Office.

• Passenger’s Declaration for Live Animal(fileSize, fileExtension)

  If you have Pet Travel request, please click and fill out the “ Passenger’s Declaration for Live Animal” form for two copies and submit them at the check-in desk.

• Medical Information Sheet (MEDIF)(fileSize, fileExtension)

  If you need special assistance or services during your flight, please notify an EVA Air's Reservations Office, so we can make the necessary arrangements.
